WHEN YOU ARRIVE When you arrive at the office, our receptionist will make a copy of your insurance card(s) and have you fill out an information form and a personal health history form. These questions are a necessary part of the examination and evaluation process. Using these forms, your surgeon will obtain basic information about your current problem and previous history. This information also enables our business office staff to complete insurance claims on your behalf. All information in your record is strictly confidential and is safeguarded according to federal HIPAA standards. FINANCIAL Please bring your insurance card with you to your appointment. If you do not have your insurance card with you, you will be required to pay in full at the time of your visit or reschedule your appointment. All self-pay patients are required to pay in full at the time of the appointment. If you are a member of an HMO or any other plan which requires a referral, please bring a copy of the referral to the office. Otherwise, it is your responsibility to see that a referral reaches our office prior to your visit if your plan requires one. You will be asked to sign a waiver or reschedule your appointment if we do not have the referral. with our staff when making your appointment. APPOINTMENTS Of necessity, our surgeons must work by appointment. Unfortunately, patients do not become ill on schedule. Surgical schedules do not always flow smoothly and emergencies happen; therefore, delays and changes in schedule are occasionally unavoidable. A sincere attempt is made to conform to schedule, or, in the event of a change, to notify you in advance so as not to disrupt your schedule. PRESCRIPTIONS & REFILLS Prescriptions and refills are only issued during office hours. Please have your pharmacy number available at the time you call our office. EMERGENCIES A surgeon is always on call for emergencies. If you have a life-threatening emergency, call 911. If you have a concern or a question for your surgeon that cannot wait until your appointment, call our office at 919-782-8210. Our staff is available Monday through Thursday from 9:00 am 5:00 pm and Friday from 9:00 am 4:00 pm. After hours, the answering service can assist you. TELEPHONE CALLS When you call the office and cannot get through to your physician, we request your patience. It is the policy of our practice not to interrupt our physicians while they are attending to a patient. Quality patient care requires the undivided attention of our medical staff. You will, however, be able to speak with an experienced nurse or the surgeon on call if your surgeon is not immediately available. TEST RESULTS It is important that our patients are well informed and obtain the results of any tests that have been performed. Pathology and cytology results typically take 48 to 72 hours. We will make a sincere effort to notify you as soon as your results become available. |
